E-Statement Agreement

Jefferson Bank of Florida offers you the ability to receive required disclosures relating to your transactions with the
bank electronically. You may elect to receive this information in paper form by calling the bank at 813-855-7500
or 727-781-7500. There is no cost to obtain paper copies of any electronic records.

Your consent to receive the information electronically applies only to the specific transaction you are conducting with the bank at this time.

You may also withdraw your consent to use or accept the records electronically at any time. In order to withdraw your consent you may contact us by e-mail, mail, or call us at the numbers listed above. Please indicate your name, address, daytime phone number, and the type of transaction you are withdrawing your consent to, but do not include any account numbers, personal identification numbers (PIN), or other information that is used for security purposes. If you should change your e-mail address in the future, please forward the updated information to the bank for our records.

In order to receive this information electronically, you will need:

• A personal e-mail address
• A computer or other device with ordinary email software
• A connection to the internet
• Adobe Acrobat Reader version 6.0 or higher (Click here for a free download.)
• A printer (optional), if you wish to print paper copies.
